Job Description

The Vice President is responsible for ensuring the health system’s virtual care, home and community-based services (including home health and hospice) and rehabilitation assets, activities and operations are achieving ever-improving clinical outcomes while lowering the total cost of care. In partnership with key stakeholders (e.g., system, divisions, senior leadership; board of directors; physicians; nursing; quality management, etc.), the VP will establish, drive, and monitor a shared vision for the health system that will accelerate the health system’s progress in achieving its mission.

What You Can Expect:

  • In collaboration with system leadership, actively supports the transformation of improving outcomes while lowering total cost of care. Serves as a thought leader with respect to the transformation of health care from fee-for-service to pay-for-value.
  • Ensure the portfolio of virtual care, home/community-based services and rehabilitation are working in a coordinated manner with appropriate oversight and direction for all provider-based activities. Facilitates effective relationships with the acute care leaders, ambulatory/service line leaders and medical staffs to promote evidence-based medical practices, care coordination, and management of care transitions.
  • Work across the system and in partnership with VP Care Management to ensure that care transitions are managed effectively and that outcomes are improved, while lowering the total cost of care by avoiding unnecessary readmissions, interruptions to care, duplication of care, medication errors, and/or the provision of care.
  • Keeps abreast of emerging models of care. Identifies, pilots, and implements new and innovative strategies, models, and partnerships to achieve better outcomes through the operations of the post-acute care division. Promotes and advocates new programs, services, initiatives, and strategies to leadership, physicians, and team members.
  • Identifies, implements, and drives efforts across all staff to standardize care, adopt evidence-based practices.
  • Leads care efforts to make SLHS a high reliability organization. With the ultimate goals of zero avoidable harm, zero avoidable complications, zero avoidable readmissions, and zero avoidable mortality.
  • Fosters an environment of innovation and embraces new uses of technology and alternative sites and methods of care delivery.

Qualifications:

  • Master’s degree in business administration, healthcare administration, or related field.
  • 10 years relevant experience.
