Are you a compassionate NY-licensed LPN or LVN with a passion for improving patient care? We're looking for dedicated professionals to join our remote team, focusing on remote patient monitoring. This role allows you to make a meaningful impact from the comfort of your own home.

What You'll Do

As a Remote Patient Monitoring Specialist, you'll:

Engage with Patients: Reach out to patients, introduce them to our monitoring program, and guide them through the enrollment process.
Provide Support: Assist patients with setting up and using their monitoring devices and the associated app.
Maintain Communication: Regularly check in with patients to ensure adherence to their care plan and address any concerns.
Monitor Data: Review patient data, track trends, and keep healthcare providers informed about patient progress and any observations.
Educate and Inform: Share relevant health information with patients and support them in understanding their condition and treatment.
About You

You're a detail-oriented and tech-savvy professional who is:

Licensed: Hold an LPN or LVN license in New York.
Experienced: Familiar with chronic health conditions and the operation of related monitoring devices.
Tech-Savvy: Comfortable with technology, including smartphones and computer platforms.
Communicative: Excellent in both written and verbal communication.
Organized: Capable of managing multiple tasks efficiently.
Self-Motivated: A self-starter who is motivated and disciplined in a remote work environment.
Preferred Qualifications

Experience in telehealth or patient monitoring.
Background in telemarketing or customer service.
Additional language skills are a plus.
