Key Responsibilities:
Patient Management: Oversee your patient caseload with a focus on resolving gaps in both clinical and non-clinical care.
Resource Coordination: Connect patients with community resources that enhance their care.
Education & Support: Provide patients with education on managing chronic conditions and support them through their healthcare journey.
Medication Management: Handle medication-related tasks including reconciliation, adherence, and refills.
Service Coordination: Ensure timely delivery of essential services like Home Health, DME, Home Infusion, and more.
Communication: Address patient inquiries and foster open communication to understand their needs.
Referrals & Scheduling: Assist with managing referrals and scheduling appointments.
What We're Looking For:
Experience: At least 3 years of direct patient-facing experience.
License: Active Multi-State/Compact License (LPN/LVN).
Skills: Proficiency in Microsoft Office Suite and excellent written and verbal communication skills.
Technical Aptitude: Ability to navigate electronic health records and other digital tools.
Preferred Qualifications:
Experience with Epic
Bilingual skills
Additional single-state licenses (LPN)

Ability to sit for extended periods

Proficiency in real-time note-taking during telephone conversations

Compensation:
