Job Description

Are you a dedicated Licensed Practical Nurse (LPN) with a passion for patient care? We're looking for a skilled LPN to join our team in a fully remote role, where you'll play a key role in delivering value-based care management for patients.

What You'll Be Doing:

Patient Care Management: Oversee patient care with a focus on closing gaps in clinical and non-clinical services.
Resource Coordination: Identify and coordinate community resources that support patient care.
Education: Provide guidance on managing chronic conditions and improve health literacy.
Medication Management: Handle medication concerns, reconciliation, adherence, and refills.
Service Delivery: Ensure timely delivery of home health, DME, home infusion, and other essential services.
Patient Communication: Address patient questions, manage referrals, and schedule appointments.
Requirements:

Active Multi-State/Compact License: Current LPN license with NLC (or LVN).
Technical Skills: Proficiency in Microsoft Office Suite.
Communication: Excellent written and verbal communication skills.
Preferred Qualifications:

Experience with Epic
Bilingual abilities
Additional single state LPN licensures
Remote Work Requirements:

Reliable high-speed Wi-Fi
A dedicated home office or HIPAA-compliant workspace
Physical Requirements:

Ability to sit for extended periods
Clear communication over the phone
Capacity to understand and take detailed notes during calls
