Job Description

PACE of Southwest Michigan is seeking a dedicated Home Care Certified Nursing Assistant. This position requires an energetic individual who prioritizes delivering the highest level of care for our participants. The Certified Nursing Assistant will perform various participant care activities, including feeding, light cleaning, and assisting with daily living tasks in a home setting. This is a full-time position with typical work hours between 8:00 AM and 4:30 PM, Monday-Friday. A personal vehicle and a valid Michigan Driver's License is required. Travel is to be expected 100% of the time. Must be comfortable with safely traveling in various types of weather conditions.

Who is PACE of Southwest Michigan?

PACE (Program for All-Inclusive Care for the Elderly) of Southwest Michigan is a 501(c)(3) organization that seeks to uniquely bridge the health and social supports required to help preserve the independence and dignity of our participants. We provide the necessary medical treatment, support, and social interaction to enable our participants to remain in their own homes for as long as possible. The care team consists of physicians, nurses, pharmacists, social workers, therapists, and transportation staff who collaborate daily to ensure comprehensive care.

PACE was born via a partnership formed by 3 local non-profit organizations; Spectrum Health Lakeland (now Corewell), Region IV Area Agency on Aging, and Caring Circle. Since PACE's inception, the organization has expanded its team to over 90 professionals and currently serves over 250 participants and their families within Berrien, Cass, and Van Buren Counties. The unique PACE model of care is centered around the belief that it is better for the well-being of seniors with chronic care needs to be served in their homes and their community whenever possible.

Specifications

Education: High School Diploma or equivalent

Experience: Six (6) months of clinical experience as a personal care aide, nursing assistant, or emergency medical technician

Number/Type of Employees Supervised: None

Licensure, Registry, or Certification Required: Nursing Assistant certification in the State of Michigan, a valid Michigan Driver's License, and a current CPR certification.

Special Training: Only acts within the scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions, hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B.

Key Responsibilities

· Provide assistance, support, and/or guidance for participants in their home environment as directed.

· Travel to participant homes to provide care.

· Assist with all ADLs as trained appropriately and directed.

· Assist with two-person assist transfers, turning, positioning, and transferring participants into and out of bed, chair, shower, wheelchair, and use of mechanical lift as needed.

· Obtain monthly/weekly weights as directed.

· Communicate any changes in function or behavior to the Home Care Supervisor.

· Assist participants on/off the bus at their homes.

· Participate in Quality Improvement activities.

· Assist with housekeeping tasks in the home, including but not limited to – sweeping, mopping, laundry, grocery shopping, etc. as directed.

· Assist with personal care tasks in the home, including but not limited to – bathing, toileting, changing, dressing/undressing, etc. as directed.

· Work collaboratively with all other departments to ensure that the needs of participants are consistently met and that a clean, sanitary, and safe environment is maintained at all times.

· Build and maintain trusting relationships with participants.

· Identify and report safety hazards and emergencies and initiate corrective action as appropriate.

· All other duties as assigned.

Skills and Knowledge

· Able to multi-task and work independently and in a team setting.

· Self-motivator and organized.

· Exposure to or previous relevant experience or training and skills in housekeeping, good health practices, observation, reporting, and recordkeeping. Additionally, skills, knowledge, and/or experience with food preparation, safe food handling procedures, and reporting and identifying abuse and neglect.

· Maintain current knowledge of and adherence to required regulatory compliance by governing entities, HIPAA, and PACE-specific regulations.

· Able to read, write, speak, and understand the English language and follow written and oral instructions

· Working knowledge of Universal Precautions.

· Effective verbal and written communication skills and good organizational skills.

· Willingness to accept and execute additional tasks as assigned.

· Able to work effectively in a collegial manner with all members of the Interdisciplinary Team (IDT).

· Able to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.

· Able to utilize basic computer skills in the workplace.

Specific vision abilities required by this job include close vision, distance vision, and the ability to focus.
